2-1-1 San Diego/Community Information Exchange (CIE) is the Alliance Healthcare Foundation’s 2011 i2 Innovation Initiative winner and pioneer of creating a shared technology platform and service enabling integrated person-centered longitudinal social and health data sharing for now 80,000 (and growing) consented-in individuals and over 60 (and growing) trusted partners in a multi-directional information and referral multi-source ecosystem that enables communities of clinical and non-clinical providers to provide proactive, holistic individualized coordinated care.
