Mission Statement: People Assisting the Homeless (PATH) is a family of agencies working together to end homelessness for individuals, families, and communities. We strive to do this by prioritizing housing while providing customized supportive services for people in need. Our agencies each address homelessness in a different way—supportive services, permanent housing development, support for homeless families, and community engagement—all of which ultimately help the people we serve make it home.

Advancing Health and Wellness: Connections Housing is a service and residential community designed to reduce street homelessness in the downtown neighborhoods by helping people who are living on the street move into permanent housing. Connections Housing provides 223 housing units, a health center, and numerous social services—all conveniently located in one building. Connections Housing is one part of downtown’s larger strategy to end homelessness.

Description: Connections Housing’s innovative housing, health, and wellness co-location model has been proven to be effective in providing care to homeless individuals. We advance the wellness of previously homeless individuals by providing timely access to 223 units of housing, including permanent studio apartments, interim housing, and single-room special needs units, coupled with easy access to primary and preventative healthcare at our on-site health center operated by Family Health Centers of San Diego. PATH recently expanded services to offer recuperative care with 48 dedicated interim beds. We are the first agency to locally launch this type of program with the goal of reducing recurrences of homelessness and public healthcare system burdens. Residents also have access to the PATH Depot, an on-site multiservice center providing 25+ diverse supportive services, from employment to life-skills workshops, intensive case management, and connections to healthcare and public benefits.
