Join fellow funders from across San Diego County to discuss and provide feedback on a new regional community coordination system being rolled out by the award winning 2-1-1 San Diego. Utilizing the tenants of Collective Impact, this new Community Coordination ecosystem utilizes community-based screenings and vulnerability assessments across 14 domains, a shared client demographic and service record, a bi-directional referral system, care team member identification and communication tools.
More than 50 local organizations have been working together to build this transformational ecosystem to drive the social and health sector. This ecosystem is great news for customers, service providers, and community leaders alike. Key stakeholders will benefit from new data sets to identify service barriers, gaps, and duplications. Funders will also reap the rewards from a new set of metrics of which they will be able to use to inform their giving at a deeper level.
